Nail on the head! A failure to eject can be caused by by allot of things but two of the more common are the ejector going south or the extractor not tuned properly. I've always thought that if the extractor will hold a round on breach face relatively tight that was good to go. when you do have a good tight extractor you also need to make sure that the bottom edge is radiused and free from burs. You can also fool around with the ejector length as that can change the way it functions also. I had two Colt GMs in 45acp one would not function with stock length ejector and the other would work with an extended one! Go figure!
